Transitional Care Management (TCM)
TCM visits occur after a patient has been in the hospital and has been discharged home. TCM visits are a very important component of the care we offer because they help patients recover FULLY and can improve a patient’s overall health and well-being. TCM visits can also help:
- Improve patient outcomes: early follow-up can help identify complications early, manage symptoms, and improve clinical outcomes.
- Improve patient satisfaction: early follow-up can help enhance patients’ engagement and satisfaction.
- Reduce hospital readmissions: early follow-up can help catch complications early and mitigate growing issues, thus keeping patients out of the hospital.
- Reduce costs: Follow-up services can reduce the total cost of care over time as re-hospitalizations or other major setbacks are prevented.
- Improve adherence to treatment plans & reduce harm: Follow-up care can help improve patient adherence to treatment plans. It can also reduce errors and misunderstandings.
After a hospital stay, it can be easy for a patient to feel confused or unsure of the things that were done during their hospital stay. They may return home and not understand the medication changes that were made. At Cole Medical we want to work in partnership with the hospital discharge team to ensure a smooth transition home!
During a TCM visit one of our providers will go over your discharge papers with you and make sure you understand the following:
- why you were hospitalized
- any new diagnoses made
- specialists you need to follow up with and when
- any new DME that needs to be ordered
- changes in your medications and how these may affect you
- and we can answer any questions you or your family may have
